eForms Guide

eForms is an all-in-one enrollment process for the food program that eliminates paper forms for centers and providers, as well as the back-office.  The guides below are intended to walk you through the enrollment process with eForms.

1  Log into KidKare

Log in to app.kidkare.com with the same credentials you use to log in to Minute MenuHX. Then, from the menu to the left, click eForms.

2  Give your providers access to eForms

First, click Enable Providers from the eForms menu. The Enable Providers page opens. Give your providers access to the eForms feature on this page.

3  Send invitations

Click Send Invitations from the eForms menu. The Send Invitations page opens. Use this page to send enrollment invitations to parents. Parents with an email address on file automatically receive an email inviting them to update child enrollment or income eligibility information online.

4  Track invitation status

Click View Status from the eForms menu. The View Status page opens. On this page, you can track enrollment progress for each of your providers. The number of invitations display, as do the total sent, not started, in progress, submitted, and so on. Use the filters on this page to view specific providers and/ or form types. You can also export this information, if needed.

5  Review forms

Click Approve & Renew from the eForms menu. The Approve & Renew page opens. As forms are completed, they are added to this page. Click a child’s name to view their updated enrollment details. Compare the old forms to the new forms, check parent signatures, and ensure that everything is in order. If forms need changes, you can send them back to parents for revision. If they do not, you are ready to approve and renew.

6  Approve & renew

Use the Approve & Renew page to approve enrollments, renew enrollments, or approve and renew enrollments in one step. Enrollment information is not updated in Minute Menu HX until you renew enrollments.

Note: In most cases, it is best to wait until your current claims are processed before you renew enrollments. For example, if your new enrollment start date is 10/1, you should wait until the September claim is processed before renewing enrollments in HX. You can approve enrollments and later return to renew them.

1  Log into KidKare

2  Give your centers access to eForms

First, click Enable Centers from the eForms menu. The Enable Centers page opens. Give your centers access to the eForms feature on this page. Depending on your preferences, this will allow your centers to send enrollment invitations, track enrollments, have parents complete eForms on site, and so on.

3  Send invitations

Click Send Invitations from the eForms menu. The Send Invitations page opens. Use this page to send enrollment invitations to parents. Parents with an email address on file automatically receive an email inviting them to update child enrollment or income eligibility information online.

4  Track invitation status

Click View Status from the eForms menu. The View Status page opens. On this page, you can track enrollment progress for each of your centers. 2  Track invitation status

Click View Status from the eForms menu. The View Status page opens. The number of invitations display, as do the total sent, not started, in progress, submitted, and so on. Use the filters to display only those invitations you need to view.

3  Working with parents/guardians

The following are common situations you may encounter while using eForms:

  • Parents/guardians do not receive the initial In this case, advise parents/guardians to check their spam/junk folders. You can also update their email addresses and re-send invitations. For more information, see Update Parent Email Addresses and Resend Invitations.
  • Parents/guardians do not have an email address at If this is the case, you can open the forms and have the parent complete the forms onsite. For more information, see Complete eForms On-Site.
  • Parents/guardians complete enrollment forms on paper.  Simply mark the form as manually completed in KidKare and send the form to your sponsor. For more information, see Mark Forms as Manually Completed.

4  Review forms

Click Home from the menu to the left and select My Kids. Then, click the down arrow next to Add Child and select Send eForms. The Enroll Child page opens. Enter basic information about the child and click Send Invitation to send an enrollment invitation to the child’s parent/guardian.

Click eForms from the menu to the left. The eForms page opens and displays your pending invitations. It includes the child’s enrollment status, the invitation status, and any pertinent dates. You can filter to the invitations you wish to view.

3  Working with parents/guardians

4  Approve eForms (If Required)

If your sponsor requires you to approve forms before submitting them, you must review and approve forms before submitting them. Forms that you approve receive a status of Submitted (Site).
